Health Benefits - 13 - Dental

Dental benefits are available to you and your eligible dependents through United Concordia DPPO or Delta Dental DHMO.  

The DPPO has an in-network benefit, but also allows out-of-network services.  There are no primary care dentist requirements, but there is a plan maximum of $2,500 per person per plan year.  Orthodontia services are available only to eligible dependent children under the age of 26.  There is an annual deductible per plan year with the DPPO Plan, which is $50 for Individual or $150 for family.  

The DHMO is an in-network provider benefit, which means that all services must be rendered by a participating provider.  A primary dentist must also be elected.  Orthodontia services are available to the employee as well as their eligible dependents.  There is no annual deductible or plan maximum with the DHMO Plan.
