Health Benefits - 10 - Medical EPO

The Exclusive Provider Organization Plan (EPO) is available with CareFirst BlueCross Blue Shield or United Healthcare and each plan provides a national network.  The EPO Plan is an in-network provider plan.  This means that all services must be rendered by a participating provider or facility unless deemed an emergency.  A participating provider or facility is one that is under contract with the insurance company.  Major medical services are covered at 100% and there is no deductible.    A co-pay applies to Primary Care and Specialist visits.  Many preventive care services are not subject to a co-pay.  Primary care physician requirements vary by plan.  Refer to the Benefits Guide to verify which plans have a primary care requirement, and for a list of services that are not subject to a co-pay.
