Health Benefits - 17 - LTC-Prudential

Long Term Care (LTC) Insurance is available through The Prudential Life Insurance Company of America.  Long Term Care provides assistance for someone with severe cognitive impairment or the inability to perform Activities of Daily Living.  With a qualifying chronic illness or disability, services may be provided at home or in a facility, and care may be provided by a professional or informal caregiver, such as a friend or family member.  Employees who enroll within the first 60-days of employment have guaranteed coverage, which means that medical history is not required in order to be approved for coverage.  Coverage is also available to dependents and family members; however Evidence of Insurability will be required, which means medical history will also be needed.  Premiums are based on the level of coverage elected.  Enrollment for LTC is either online or through the Customer Service Center.  Customer Service is available Monday - Friday from 8:00am - 8:00pm EST at 1-800-732-0416.  Detailed information regarding premiums and coverage options is available online at www.prudential.com/gltcweb/maryland, and general information is available in the Benefits Guide.
