Health Benefits - Dependent Documentation

The following documentation will also be required for enrollment:

Spouse - A copy of the official state marriage certificate. Official certificates are those signed by the Clerk of Court, State Registrar, or other court official. Certificates signed by a minister, pastor or other clergy member are valid, but are not the official version.  An official version may be obtained through the municipality  where you were married or online at www.vitalchek.com.

Biological children - a copy of the child's official birth certificate is needed.  The certificate must show lineage

Pending Adoptions -  a copy of the notice of placement for adoption on the adoption agency's letterhead or a copy of the court order placing the child pending final adoption

Completed Adoptions - a copy of the final adoption decree signed by a judge or a state issued birth certificate indicating the employee as the parent

Step-Children - a copy of the child's official birth certificate with the name of the spouse of the employee as the child's parent, and a copy of the employee's official state marriage certificate

Other Child Relatives - a copy of the child's official birth certificate, proof of permanent residency with enrolled employee.  In addition, legal wards will need to have a copy of the court order signed by a judge, and grandchildren, step-grandchildren and other child relatives will need birth official birth certificates verifying proof of blood relation or marriage
